Community Counts gives you a chance to learn more about the people and the organizations that have a positive impact on our community. This week we feature the Children's Health Improvement Program (CHiP).
Judy Smith joined CBS19's Bo Sykes to talk more about the program, provides services through a home visiting model using a case management approach. Services are provided by a CHiP team, made up of a nurse and family support worker, who work in partnership with the family and service providers to help each family reach their full potential.
