Community Counts gives you a chance to learn more about the people and the organizations that have a positive impact on our community. This week we feature the Jefferson Area Board for Aging and its Chronic Disease Self-Management Program.
Elyse Thierry joined CBS19's Bo Sykes to talk more about the program, which consists of a series of six workshops that meet weekly for 150 minutes.
The program is designed to help people suffering from chronic disease to take control and break the cycle of symptoms. The goal is to give participants the skills they need to take that control.
The program is primarily designed for senior citizens, but anyone can join.
Participants will meet weekly and develop doable action plans in the interactive workshops. The program starts in February and people interested in attending should call 434-817-5271.
