State regulators have cited a Chesapeake golf course for an accident that killed its grounds superintendent.

State regulators have cited a Chesapeake golf course for an accident that killed its grounds superintendent.

The Department of Labor and Industry is proposing $16,800 in civil fines against Cahoon Plantation.

Forty-two-year-old Michael Costa of Camden, N.C., was crushed by a turf sprayer on Aug. 1, 2011 on the golf course's eighth hole.

The labor department says Costa didn't engage the sprayer's parking brake or block its wheels when he left the engine running on a 15-degree incline.

The department faulted Cahoon for improper use of the turf sprayer and inadequate training.

Cahoon golf operations director Dan Shea tells The Daily Press that the golf course is in talks with the department and doing everything it can to prevent such accidents in the future.
